This tool helps recognise some of the factors that are getting in the way of the focus person.
It actively encourages the focus person and people involved in the development of the support plan to think of solutions to get past these barriers. This tool would be great to complete with the focus persons Circle of Support initially.
This template comes with guidance, an example and the blank template for you to use.
