This tool captures how to make big or small decisions. When necessary it may mean other people complete this on behalf of the focus person, keeping the focus persons best interest at heart.
It is good practice to have the focus person involved when creating this agreement.
You could complete two of Decision Making Agreements one for making decisions when the focus person is in good health and one for if the focus person goes through patches of bad health.
It is also good to use this tool for future decisions especially if the focus person has a progressive condition.
This template comes with guidance, an example and the blank template for you to use.
