Supporting people to feel hopeful, identify their own purpose and confidently take action

Support Planning

A Support Plan is a plan that tells other people about how a person wants to be supported in order to live their life in the way that they choose.

A Support Plan typically covers daily routines and support requirements for specific tasks or activities. If a person is accessing social care or health funding to cover the cost of their support, the Support Plan will need to be agreed and signed off by the funding authority, with clear costs shown against each item in the plan.

At Imagineer, we use the tools and processes of Person Centred Planning to help people build their own Support Plans.

Our team are all experienced in Person Centred Planning facilitation and Support Brokerage to help the person understand their rights, and help them to explore which support options will be best for them. We don’t simply explore what is ‘available’. We explore how we might create something bespoke for the person, based on what they have expressed they would like to do with their life and how they would like to live and be supported. In this way, the person is able to develop a creative plan for their support which really reflects them as an individual.
