Supporting people to feel hopeful, identify their own purpose and confidently take action

Person Centred Planning

Person Centred Planning is a way to plan with people, to help them think about what they want to change or achieve in their own lives.

The person is in charge of their own plan and in control of how it is created.

A Support Broker from Imagineer can help the person to create their plan.

With a Person Centred Plan, we start by asking the person questions about:

  • What is important to you?
  • What matters to you?
  • What do you want to be doing in the future?
  • Where would you like to be living in the future?
  • How do you want to be supported?

The person can ask anyone who is important in their life, like family and friends, to be involved in their plan.

After we have talked about the person’s ideas, we can explore how to make them happen, and create an action plan so that everyone understands what to do.

The person can choose how they want to work with the Support Broker to create their plan.

We can meet the person face-to-face, or online to talk about their ideas.

We will explore different ways the person can present their plan to find what works best for them. This could include video, audio, photographic or writing.

The Support Broker will help the person to decide what they want their plan to be like and how to work on it together.

The plan belongs to the person and they can choose who to share it with.
