Supporting people to feel hopeful, identify their own purpose and confidently take action

Life Planning

Life Plans are documents developed with (and for) people with Learning Disabilities and Autistic People in long stay mental health hospital settings. Life Plans can help to inform the development of a service specification for care, support and accommodation to support the person outside of the hospital setting and back in their community, as well as articulating what a good life means to the person in terms of their aspirations, hopes and goals for the future.

A Life Plan is a Person Centred document which is written from the focus person’s own perspective and helps professionals, practitioners, housing and support providers understand how to build a truly person-centred pathway towards life in the community.

Life Plans can help to form an outline for discussion in Care (Education) and Treatment Reviews (C(E)TRs), and 117 discharge planning meetings.

We also offer ‘All About Me’ training which can be developed alongside the Life Plan to support the introduction of a support team around the person when they are moving back to the community.

We offer a discounted package if you purchase a Life Plan and All About Me training together.
